How We Make a Diagnosis

...Things to Give Dr. Glazer Before Your First Visit

It helps us to have as much information as possible to give you the most information.

If you are seeing us for Memory Changes,

please do your best to obtain 3 things.  Dr. Glazer usually cannot make a diagnosis without these 3 things:

  • Blood work for any medical contributors to memory changes, including CBC, CMP, B1, B6, B12, methylmalonic acid, folic acid, ammonia, HIV, and syphilis testing.

  • Urinalysis with urine culture.

  • A brain MRI without contrast, preferably with Neuroquant analysis (please contact us to ask how to obtain this), please bring both the report and the CD with images

  • A Neuropsychology evaluation

Please note that sometimes a diagnosis cannot be made on the first visit because additional testing may be needed depending on your individual case.  In that case, a diagnosis will occur on a follow-up visit.

It is also not essential but would be helpful to have:

  • A recent Progress Note from your PCP

  • Any notes or testing done by previous Neurologist(s)

Insurance Information

Dr. Glazer is committed to high quality Memory and Neurological Care.  She provides services that may not be covered by some insurance plans.  For this reason, the family pays out of pocket initially. For all plans except for Medicare Part B, the patient or family can submit for reimbursement to their insurance on their own.

Our Rates

For non-Medicare Part B patients (as in people who do not have the Medicare Part B red, white, and blue card):

New Visit $540 includes 1 hour visit* 

Return Visit $275 includes 30 minute visit* 

Return Visit $540 includes 1 hour return visit* 

​

*There may be additional time needed for other items at a rate of $13 per minute.  These may include:

  • Laboratory Test Review

  • Imaging Review 

  • Neuropsychology Report Review

  • Medical Record Review 

  • Discussing your Case with other Physicians and your Care Team

  • Text Messaging Dr. Glazer 

  • Phone Calls with Dr. Glazer 

  • Paperwork filled out by Dr. Glazer 

  • Prior authorizations for medications

There are no fees for cancellations, rescheduling, prescription refills, or insurance reimbursement questions.

There is a discount for people with Medicare Part B (the red, white, and blue card):

New Visit $395 includes 1 hour visit* 

Return Visit $200 includes 30 minute visit* 

Return Visit $395 includes 1 hour return visit* 

*There may be additional time needed for the other items listed above at a rate of $12 per minute, for Medicare Part B patients.

We send your Progress Note to you after each visit, and you can submit it for reimbursement to your insurance company, except in the case of Medicare Part B.
